POSITION TITLE: ADP Case Manager
RESPONSIBLE TO: ADP Manager

RESPONSIBLE FOR:
This full-time, non-exempt position supports all aspects of the Any Door Place (ADP). The ADP is a drop in day center program offering services to individuals and families experiencing homelessness in Yamhill County, OR.
Our goal at ADP is that all households utilizing the program would be able to access all YCAP services and those in the surrounding area. This position will work closely with households that need wrap around support and navigating the various resources in the Yamhill County area.

WORK ENVIRONMENT:
This position will work closely with the unhoused population in both office setting and close quarters.

ESSENTIAL DUTIES:

1. Create a positive team atmosphere providing quality service to clients, staff and volunteers through active listening, positive problem solving, and timely responses.

2. Assess client needs and provide referrals to agency programs or to other community resources as appropriate.

3. Provide case management for clients in agency programs including such things as applications to services, goal setting, advocacy, and referrals to partner agency services.

4. Provide Triage Case Management style case work. Such work will utilize diversion tactics by referring to other partners in the area.

5. Provide timely report, input data, and ensure all files and reports are accurate and grant standards are met.
Assist with data for monitoring site visits.

6. Assist with agency activities around homelessness including such things as the Homeless Count and the
Community Connect.

7. Requires completion of specialized trainings that meet ADP requirements. This includes but is not limited to: De-Escalation, Trauma informed care, Coordinated Entry, Non-Violent Communication,
Harm Reduction, Fair Housing, Domestic Violence Support Mental Health first-aid, and SOAR.

8. Collaborate with other agencies, businesses and volunteers to ensure that agencies’ programs coordinate to provide the best use of resources.

9. Local, in county driving to client and partner agency locations
10. Other duties as assigned.

KNOWLEDGE, SKILLS, & ABILITIES

• BA degree in sociology and/or equivalent experience
• Knowledge of computers, software programs and databases
• Skill in verbal and written communications
• Skill in problem solving and team work
• Ability to maintain confidentiality of client and organizational information
• Ability to work independently on multiple projects in a timely fashion
• Ability to work with diverse people, organizations and situations
• Ability to work with a flexible schedule
• Valid driver’s license and ability to pass drug/alcohol & background check and insurance check of driving record

PHYSICAL DEMANDS
The physical demands described represent those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee may be required to sit, stand, kneel, walk and/or bend for extended periods of time. May lift and/or move up to 20 pounds.

Job Type: Full-time

Pay: $21.00 per hour
