We are a Federally Qualified Health Center (FQHC) and have been providing care to Yolo County residents since 1972. Like other community health centers, we provide care to anyone that walks through our doors and are located in all major cities within Yolo County (Davis, Woodland and West Sacramento) and provide services throughout Yolo County. The community health center movement was born out of the civil rights movement in the 1960s with the mission to provide healthcare in communities where there are few or no options for care. Our mission is rooted in the philosophy that health care is a right, not a privilege. We serve and provide care for those disproportionally impacted by social determinants of health.

SALARY:

  • Exempt position
  • The pay range for this position is:
    • ASW/MFT/APCC: $66,50/yr.-$68,640/yr. (Based on 1.0 FTE
    • LCSW/LMFT/LPCC: $78,936/yr.-$95,000/yr. (Based on 1.0 FTE)
    • Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.
  • May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).

OVERVIEW: The Behavioral Health Clinician reports directly to the Behavioral Health Perinatal Manager. The position will provide in-home/in-community and in-clinic engagement, service navigation, and individual and family counseling to Pregnant/Parenting clients enrolled in early intervention in-home visiting and support programs. CommuniCare Health Center's mission emphasizes supportive, nonjudgmental care, preventive care, health education and community referrals to address the broad spectrum of health and psycho-social concerns of clients served.

SCHEDULE: 40 hours per week. Monday-Friday 8am-5pm at Salud Clinic in West Sacramento as well as in-home and in-community services. Some evenings shifts may be required. Travel between CommuniCare facilities required. Ability to meet clients in-home and in-community required.

RESPONSIBILITIES:

The following reflects managements definition of essential functions for this job but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or other reasons.

  • Engages clients in appropriate services, utilizing effective rapport building and motivational interviewing skills.
  • Conducts screenings and intake assessments for clients utilizing appropriate assessment tools.
  • Works with clients to development client centered treatment plans by establish long- and short-term goals and action steps to achieve goals.
  • Provides individual, group, couples, and family counseling and psychoeducation to clients in-home, in-clinic and in-community through the Road to Resilience Grant.
  • Supports client success and stabilization in treatment services by providing case-management, referral connection, monitoring progress and on-going engagement efforts.
  • Completes all client information and documentation in a timely manner according to CCHC, First 5, and OCAP guidelines and standards, including required pre/post screening and data collection tools.
  • Collaborates with multidisciplinary team to identify best practices for services provided.
  • Responsible for detecting emotional, psychological, and physical danger signals with a non-judgmental approach and offering feedback and support to clients.
  • Provide triage, crisis response, and support services to medical patients experiencing mental health symptoms, collaborate and communicate with medical staff regarding care coordination.
  • Responsible for communicating with and case coordination with referring agencies and other collaboratives including but not limited to First 5 Yolo County, Yolo County Childrens Alliance, Yolo Crisis Nursery, Yolo County Child Welfare Services, and Healthy Families Yolo County.
  • Attend collaborative with multidisciplinary team meetings with internal and external providers.
  • Responsible for remaining within the ethical standards established in the field, including those of confidentiality.
  • Responsible for explaining and enforcing all rules and regulations associated with clinic treatment policies.
  • Compile and report statistics as required.
  • Attend clinical supervision and weekly staff meetings. Attend training/conferences as arranged.
  • Responsible for maintaining the standards as listed in the CommuniCare Health Centers Personnel Policies and Procedures Manual, and any other standards which may apply to this position.
  • Responsible for maintaining current license/registration in good standing, submit all training certifications and CEUs to Human Resources, must attend ASAM Training 1x per year.
  • Other projects as may be assigned.

Education: Masters degree in social work/Marriage and Family Therapy/Mental Health and registered with the Board of Behavioral Sciences required. Minimum of one year experiencing working with clients with perinatal mood and anxiety disorders, involved in addiction treatment and/or involved with Child Welfare Services preferred.

Expectations: Bilingual English/Spanish PREFERRED. Attendance and punctuality is necessary. Ability to work well with a multidisciplinary team. Ability to problem solve and be self-motivated. Understanding substance use and approach/best practices. Must have a commitment to excellence and high standards. Must have effective and flexible communication skills, strong organizational, problem-solving, and analytical skills. Must have ability to manage priorities and workflow. Required to have versatility, flexibility and a willingness to work within clients homes and community. Expected to have strong interpersonal skills and have the ability to engage clients effectively. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.

Physical Requirements: Required to sit for prolonged periods. Able to bend and able to lift 15-25 lbs. Hearing and vision are required. COVID VACCINATION REQUIRED
