Description: Rocky Mountain Primary Care is seeking a full-time Patient Care Coordinator at its Lakewood location. The Care Coordinator is an integral part of the patient’s care team which includes provider, medical assistant, receptionist, referrals coordinator, and behavioral health. The care team is responsible for the delivery of patient-centered care and works toward improving the health of patients within the practice who are at high risk for adverse outcomes. You will provide care coordination for patients with complex needs, as identified through risk stratification and provider referral, by collaborating with the care team, specialists, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services.
The Care Coordinator is responsible for:

  • Supporting the provider’s clinical plans of care by connecting patients with services that will optimize the patient’s health status.
  • Collaborating with the care team to integrate evidence-based clinical and preventive guidelines to develop individualized, patient-facing care plans.
  • Identifying and understanding various services available within the community and connecting patients to these services where appropriate.
  • Providing outreach, disease management, education, and motivational support to encourage positive outcomes, smooth transitions, and healthy lifestyle choices that will reduce long term effects of chronic illness.

Depending upon specific qualifications of the individual and the needs of the practice, the Care Coordinator may be involved in group education, scheduling triage, tracking referrals, researching community resources, and assisting with general paperwork related to patient care support (i.e., oxygen orders, home health orders, prior authorizations for medications, prescription refills, etc.)
This position will function in a FAMILY PRACTICE setting. It is full time, Monday through Friday, and offers some flexibility with hours ranging from 7:00am to 5:30pm.
Requirements:

EDUCATION/EXPERIENCE/LICENSURE*:

  • REQUIRED: 5+ years medical practice or other clinical work experience
  • REQUIRED: Experience with electronic health records
  • PREFERRED: Graduate of an accredited RN program or Medical Assistant certification from an accredited program
  • PREFERRED: National Certification obtained from an accredited organization (registered or certified credentials acceptable)
