Position Summary Information

General Information
The Healthcare for the Homeless Suitcase Clinic, a healthcare initiative dedicated to improving the health of the local homeless population, is accepting applications for a full-time Care Coordinator. The Care Coordinator (CC) advocates for patients’ needs by helping them navigate the healthcare system and communicate effectively with healthcare providers, social services and community organizations. The CC also works closely with healthcare providers and case managers to improve patient care and outcomes. They promote health within a community assisting individuals to adopt healthy behaviors and by advocating for programs that promote, maintain, and improve individual and overall community health.

The Healthcare for the Homeless Suitcase Clinic is a part of JMU’s Institute for Innovation in Health & Human Services ( IIHHS ). Duties and Responsibilities
  • Establish trusting relationships with patients while providing general support, encouragement and motivation to be engaged participants in their health.
  • Act as a client advocate and liaison between the client and community service agencies; facilitating communication and coordination of services between providers and arranging referrals as appropriate.
  • Act as a healthcare navigator to help clients access needed health-related resources (including accessing transportation, if needed).
  • Provide follow-up to assist clients in understanding their care plan while empowering them to take an active role in creating and implementing it.
  • Assist clients with completing relevant applications and registration/enrollment forms.
  • Document client care coordination activities and tracking clinic metrics (database).
  • Continuously expand knowledge and understanding of community resources and maintain positive working relationships with service providers.
  • Provide onsite assistance with each Suitcase Clinic; manage and order supplies.

Qualifications
  • High School Diploma or equivalent.
  • Effective verbal and written communication skills.
  • Experience working in a healthcare setting with people experiencing homelessness.
  • Basic computer skills required (includes Microsoft Word, Access and Excel).
  • Knowledge of community resources appropriate to needs of clients.
  • Ability to work collaboratively and to use resources to problem solve.
  • Self-motivated with excellent time-management and organizational skills.
  • Knowledge of some medical terminology preferred.
  • Must be available to travel, and to work variable hours ranging from 8am-9pm determined by clinic schedule.

The Annual Security and Fire Safety Report contains information regarding campus security and personal safety including topics such as: crime prevention, the university police law enforcement authority, crime reporting policies, disciplinary procedures, protocols for the immediate notification of students and staff upon confirmation of a significant emergency on campus and other matters of importance related to security and safety. The report includes statistics for the most recent three-year period of reported crimes that have occurred on campus; in certain off-campus buildings or property owned or controlled by JMU or its affiliates; and on public property within or immediately adjacent to and accessible from the campus.

The Annual Security and Fire Safety Report includes campus housing fire statistics for the most recent three-year period, a description of each on-campus student housing facility fire safety system, the number of fire drills held during the previous calendar year, policies or rules on portable electrical appliances, smoking, and open flames in student housing; and procedures for student housing evacuation in the event of fire along with other fire safety information.
