Care Coordinator: Crossroads Continuum [Position # 1110]

Crossroads Continuum provides transformative and imperative service to those who have made the empowered decision to undertake lasting recovery, sobriety and community rehabilitation.

This Care Coordination role has a clinical backbone and provides screening, education, engagement, referral and linking services. It supports SWFL residents who have substance use disorders and co-occurring treatment needs in accessing and engaging in services.

POSITION DETAILS:

- Coordinating screening, treatment, and supportive services to promote recovery from substance use disorders.

- Providing referral, advocacy, and case coordination services with goal of linking individuals to appropriate treatment levels of care.

- Linking responsibilities, accessing/utilizing other services, contacting partner agencies, and coordinating delivery of services.

- Assuming an advocacy role for clients, including advocating for admission to other services.

- Functioning as part of a multi-disciplinary team.

- Assisting Supervisor to manage wait lists and referrals.

- Supporting all Clinical Staff in accelerating timeliness of service provision.

- Participating in treatment team meetings and assisting with client interventions to support recovery and linking with treatment.

- Providing Tele Health wellness calls and coordinating internal/external services.

- Updating outside referrals and thorough electronic medical records completion.

- Developing strong community partnerships and collaborations to engage referrals and transitions in care.

TO BECOME A MEMBER OF OUR CROSSROADS CLINICAL TEAM:

- Bachelor's Degree from a regionally accredited college or university in psychology, social work, behavioral science, or related area of study OR minimum of 5 years of related experience = Required

- Spanish Bilingual = Preferred But Not Required

- Experience working with substance use disorders, mental health, and co-occurring disorders = Required

- Knowledge of recovery and relapse prevention = Strongly Preferred

- Exceptional and intuitive track record in assessing client needs = Required

- Efficient, empathetic, and evocative communication skills = Required

- Demonstrated and experienced-based client service abilities = Required

- Valid FL Driver's License and clean driving record = Required

- Accurate reporting and record keeping skills = Required

- Staying compliant on all trainings and immunizations = Required

- Ability to pass a Level II DCF/AHCA Fingerprinting, Drug Screen & Local Criminal Check = Required

LOGISTICS & COMPENSATION:

- Full-Time Salaried/Exempt position
~ Target standard business hours, Monday-Friday
~ Some schedule flexibility needed to meet client needs
