Program Description
This project is funded by the BSCC (Board of State Community Corrections) it is a project that will case manage participants from the justice-involved population, providing Reentry Peer Advocates for system navigation, mentorship, advocacy, and case management, and 24/7 211 Contact Center support for information, referral, and warm handoff to needed resources. It will expand capacity of our Reentry Peer Advocates service by 2 fulltime positions, one dedicated to Riverside County and one dedicated to San Bernardino County. It will assist the clients with housing, employment, medical, food insecurities and any other needs. This program will deliver legal clinics and collaborate with members and partners in our community.
Position Overview:
This position will expand Warm Handoff and Reentry Peer Advocacy in the Inland Region of Southern California. A dedicated Reentry Peer Advocates for Riverside and San Bernardino Counties will case manage, system navigate, refer warm handoff, and facilitate peer mentor support groups for reentry clients. Housing services to end homelessness or stabilize existing at-risk housing as well as coordinate employment services to improve income outcomes. Assist clients to ensure medical coverage. This multi-pronged approach reduces recidivism, improves quality of life and success in the community after incarceration, and develops positive peer role mode. This position will report to the Program Manager under the purview of the 211 Director. No contract is implied; this position is “at-will” and employment may be terminated at any time by the employee or the employer. This position reports to the Program Manager. Essential Functions (90%)
- Answer calls from the justice involved population and do assessments
- Provide Resources needed to reduce barriers and assist clients into integrating back to the community.
- Enroll individuals in Medical, Cal Fresh, appropriate career and housing programs.
- Connect clients by providing warm-hand offs to the appropriate partners with the appropriate resources.
- Peer advocates will organize and facilitate a peer mentor support group for the participants to meet quarterly and share experiences and solutions to keep encouragement and fresh ideas for the participants.
- Peer mentors will all assist in organizing legal clinics, to assist with expungement, tattoo removal etc. For the participants
- Collaborate with partners to ensure the resources and referrals are accurate
- Follow-up with clients to ensure successful outcomes of resources.
- Ensure all details are logged and entered our case management system
- Make appropriate presentations within the community.
- Conduct and attend outreach events
-Case Manage clients throughout the entire process and terms of the grant -Ensure high-quality data collection of every single crisis line contact via iCarol /inContact and Opexa -Participate in a positive and inclusive working environment for team members -Ensure compliance with Mandated Abuse Reporting laws, HIPAA .

- Provides expertise on peer support groups to community leaders
Qualifications
- Must be a peer of the justice involved community
- Proven employment track record of at a minimum 2 years.
- CDAAC Certification preferred
- Bi-lingual in Spanish helpful
- Strong verbal communication
- Case Management Experience helpful
- Computer experience helpful
- Motivational Interviewing Certification helpful
- Must have reliable transportation

Salary Range and Benefits
$25.00 HR, Benefits include vision, dental, medical, life, AD&D and long-term disability insurance, 11 paid holidays, accrued vacation and sick leave.
