• Must Live in Milwaukee county or within 30 miles of the city
• Provide Prenatal and Childcare Coordination as well as case management services to at risk pregnant women and women with infants
• Recruit and enroll pregnant women and women with infants/members/outreach
• Complete assessments and care plan development
• Ongoing care cordination and monitoring
• Provide personalized care to meet the individual needs of clients
• Connect member with necessary resources and aid in the communtity
• Assist with individual and/or group health and nutritional training
• Provide advocacy and advice, enable members to break barriers, be committed and supportive to help clients establish more abundant life outcomes for their infants, children, and families.
• Ability to complete home visits, as well as zoom, email, phone etc., However, some Face-to-Face visits will be required to be completed.

Job Types: Full-time, Part-time, Contract

Pay: $20.00 - $21.00 per hour
