Clinic Supervisor Job at St. Paul's Senior Services
St. Paul’s Senior Services is excited to announce an amazing opportunity to be part of San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community.
The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Job Summary: Under the supervision of the Center Director, manages flow of clinic leading all staff in provision of medical and nursing care, ensuring high quality standards. Coordinates services with primary care team, acts as primary liaison.
Triages, and assess participants as needed. Acts as gatekeeper for daily clinic scheduling. Provides skilled nursing care to participants, implements orders, develops appropriate plans of care, provides participant, caregiver, family, and staff teaching and communicates participant’s condition to caregivers, primary care, and the interdisciplinary team. To ensure continuity of operations assumes the Center Directors responsibilities in her/his absence.
Qualifications & Requirements: • Education - Graduate of a school of professional nursing program required. BSRN strongly preferred. Current California Registered Nurses License and CPR certification required. Shall be at least 18 years of age.
