Clinical Pharmacist- AntiCoag/TOC Job at BALTIMORE MEDICAL SYSTEM, INC.
The Clinical Pharmacist (CP) maintains a positive working relationship with the clinical teams and pharmacy teams to ensure effective clinical/pharmaceutical outcomes via medication therapeutic management. The clinical pharmacist will also work to empower patients to actively participate in their own clinical decision-making. The CP will work with the Director of Pharmacy, Pharmacy Managers, and other Clinical Pharmacist(s) to ensure that pharmacy operations are running efficiently. The CP will provide patient care that optimizes the use of medication and promotes health, wellness, and disease prevention. Frequent communication outside of the work group is required to resolve disagreements, reach agreements and clarify issues/processes. Responsibility exists to ensure that internal customers are satisfied with service. External communication involves the development and maintenance of relationships with major organization providers.
Schedule/Hours:
Mon-Fri; 8:30-5:00 PM
- PharmD
- Completion of PGY1 or BCACP certification
- MD Board of Pharmacy Licensure
- MD Board of Pharmacy Immunization
- Certified CPR
- MTM Certification
- 5 years pharmacy experience preferred
- Ambulatory Care Setting
- Knowledge of Coumidin therapy highly preferred
