The clinical pharmacist will establish and maintain medication reconciliation programming for Next Generation Accountable Care Organization (NGACO) and other at-risk and/or value-based patient populations.
Provide clinical pharmacy services to support Affinia Health Network's Medication Therapy Management (MTM) strategy to ensure patient education on topics of medication safety, side effects and interactions.
Utilize data to proactively identify patients who would benefit from pharmacy intervention and coordinate with providers and/or care management to develop, document, and execute drug therapy plans using safe, effective and economical medication therapy options.
What the clinical pharmacist will do:
- Perform medication reconciliation assessments for patients, collaborating with patient's PCP and care manager, to optimize the patient's drug therapy regimen.
- Creation and documentation of standard work for clinical tasks requiring documentation the Electronic Medical Record. Coordinate this work with care management as well as ambulatory pharmacy teams to ensure consistent practices.
- Evaluate drug therapy through clinical assessment, direct patient/caregiver involvement, and review of medical chart. Identify and take specific corrective action for drug-induced problems. Document findings and recommendations to appropriate team member and in appropriate medical record.
- Review and screen patients utilizing available data feeds to identify areas of opportunity for pharmacy intervention, with an emphasis on cost containment at both the patient and population level, while maintaining efficacy of therapy and safety of medication regimen.
- Assist providers and care managers in the selection of the most cost effective, evidence-based regimen for a particular patient.
What the clinical pharmacist will need:
- Doctor of Pharmacy, required.
- Pharmacist license and controlled substance issues by Michigan Board of Pharmacy.
- ASHP PGY-1 residency or two years of related experience, minimum.
