Here at Horizons Living and Rehab Center, we are looking to add a social worker to our staff to help our residents live their lives fully and richly. This full time director position will work in concert with our nursing team to help our residents live happy and successful lives. Some of your responsibilities will include:

• *Participating in family conferences
• *Helping to plan discharges back to the community.
• * Assesses all new residents to obtain history, establish specific goals and to complete the appropriate section of the Minimum Data Set (MDS). Writes progress notes according to established schedules.
• *Assists in providing education to staff on resident rights, dignity and privacy.
