ECM Care Coordinator Job at Peninsula Healthcare Connection
Peninsula Healthcare Connection is seeking an ECM Care Coordinator with ECM experience to make $55-75,000 (salary depending) to try to change the experience of the unhoused and medically complex in Santa Clara County. Some Job Duties are:
- Independently manage assigned caseload.
- Provide in-person, community-based case management and care coordination services that are strengths-based, client-centered and trauma-informed.
- In coordination with clinical ECM staff, complete comprehensive assessment to evaluate needs and create appropriate health action care plan.
- Responsible for overall care coordination and communication between multidisciplinary providers to ensure provision of services and implementation of care plan.
- Assist clients to access medical, mental health and substance use treatment providers, including accompaniment to appointments as needed.
- Complete documentation, data, and billing requirements in a timely manner per published procedure and timelines.
