This position is for a CCM Enrollment Specialist for a busy neurologist practice in Wichita Falls, TX. The CCM program currently serves hundreds of enrolled patients with the help and management of a dedicated team of professionals. The program results are outstanding. The good news is more patients would like to join our program.

Expect 8-5 Monday - Friday with weekends off, but hours and days may vary based on clinic schedule

Duties include but not limited to:

· The ability to follow a proven process that promotes effective patient education and program enrollment techniques.

· Effectively call, email and text patients, as well as off-site care team members to enroll patients in our CCM program.

· Ensure patients thoroughly understand the program goals and be able effectively explain all the available benefits to their health by enrolling in the program.

· Be able to navigate the practice EHR computer systems data to identify qualified patents for the program.

· Demonstrate a positive, professional and cheerful attitude toward patients.

Qualifications

· Excellent customer/patient service skills

· Medical practice and Electronic Health Record software experience a must

· Next Gen system software experience a plus

· Highly motivated individual

· Able to work independently to maintain goals
