WellStone is North Alabama’s most comprehensive behavioral healthcare provider serving over 13,000 annually in Huntsville and Cullman. We are a non-profit community organization that offers a wide range of services for every phase of life, from children to older adults.

WellStone Inc is looking for a well-organized and highly patient individual to work with Nursing Home Client's utilizing: Therapist's clinical direction, Client treatment plan, or Nursing Home feedback. Must have the ability to document electronically, be proficient in Microsoft 365, and understand how to save documents on the computer. Full-time, first shift. Flexible hours.

What we're looking for:

• High school Diploma or equivalent
• Proficient with Microsoft Office
• Provides mental health services in assigned nursing homes.
• Creative individual with artistic or musical abilities; preferred.

Duties and Responsibilities

• Deliver direct clinical services
• Ensure Basic Living Skills are provided as indicated in treatment plan
• Ensure Family Support is provided as shown on the treatment plan
• Complete administrative and clinical paperwork
• Provide clinical documentation which is present, accurate, timely, and in accordance with all regulatory standards
• Complete paperwork needed to open a case immediately upon receipt from therapist
• Complete documentation needed to bill and collect for services
• Complete reports, documentation, and nursing home audits by established deadlines
• Completes documentation in Avatar daily
• Provide program support services
• Complete documentation in Avatar on a daily basis
• Assist therapist in updating consents, getting Treatment Plan signatures, etc.
• Complete peer review as directed by supervisor
• Provides clinical services in other clinical settings as needed.
• Maintain the Geriatric Services section in the nursing home medical record
• Maintain Spirit, Quality, and Productivity expectations
• Maintain quality to comply with WellStone standards
• Maintain the spirit expected by WellStone
• Maintain an acceptable level of productivity
• Follow established Geriatric Services policies and procedure
