The Healthy Families Supervisor is responsible for providing supervision for direct service staff, reviewing progress and plans for each family in a supportive environment that also provides accountability to standards set forth by HFI, HFA and New Hope Services.

This position will provide services in the following Indiana counties: Fayette, Union, Rush and Franklin.

ESSENTIAL DUTIES:
1. Be trained and thoroughly understand the guiding principles of Healthy Families America by attending CORE Training, BASIC Training and all related trainings as requested by Manager or Director
2. Provide community referral sources with information and training on Healthy Families
3. Ensure that all staff complete Healthy Families Indiana required trainings as needed
4. Ensure that all staff trainings are documented in the Training Tracking System Database
5. Complete monthly file audits and a quarterly shadow on all staff
6. Responsible for a minimum of 2 hours of individual supervision for each full time staff member and 1 hour of supervision for each part time staff.
7. Documentation Responsibilities
a) Responsible for all staff supervision logs with weekly supervision documents to include documentation regarding programs, assessment tools, and home visits on each family served.
b) Review quarterly report information in HVTIS
c) Assign cases in HVTIS
d) Review caseload size for all staff, in HVTIS and document this in staff supervision logs
e) Ensure that Quarterly Data Clean-up is completed by SCAN established deadlines.
8. Ensure that 80% of prenatal assessments occur or within two weeks of birth
9. Ensure that 100% of assessments are completed prior to the child's three month birthday
10. Ensure that 100% of first home visits occur prior to the child's three month birthday
11. Ensure that 75% of clients receive the indicated number of home visits based on "level of service" criteria
12. Ensure that 90% of families are linked to appropriate community resources
13. Ensure that all assessments are conducted using the standardized assessment tool
14. Ensure that 80% of families are linked to medical providers
15. Ensure that 80% of children have completed appropriate immunizations
16. Ensure that 90% of children have received age appropriate developmental screening and children with a failed screening item will be a repeat screen
17. Ensure that assessment rates are maintained at a level to adhere to the program goal.
18. Monitor and maintain a caseload weight of 28-30 points for staff
19. Complete certification for First Aid, CPR, BBP and SCM
20. Must be willing to respond when off duty for emergency needs of clients
21. Attend all required Healthy Families Team Meetings and scheduled supervision
22. Participate in HF Advisory Board Meetings
23. Maintain confidentiality regarding all client contact, personnel and financial
24. Contribute to the New Hope Services, Inc. team to accomplish services area goals

QUALIFICATIONS:
1. Ability to work a flexible schedule
2. Ability to make decisions in stressful situations
3. Desire to support and strengthen families
4. Knowledge of child development
5. Must possess a valid state driver's license, proof of insurance coverage per company policy, and have a vehicle that is reliable for transportation of consumers

EDUCATION/YEARS OF EXPERIENCE:
1. Bachelor's degree in Social Services field such as Sociology, Psychology, or Social Work or other degrees deemed applicable in accordance with established service standards required
2. Two years professional experience with children and families preferred
