HOSPICE RN CASE MANAGER - Immediate Hire Needed!!
Are you a Home Care RN looking to get into Hospice or an experienced Hospice RN looking for a change?
We are seeking an RN for a Hospice Care Service serving Philadelphia, Bucks, and Montgomery Counties. Our client is seeking an experienced Hospice RN or a motivated Home Care RN who is looking to gain knowledge and experience in Hospice.
- Salary range: $80,000 - $100,000 based on experience
- Autonomy and Flexibility
- Willing to train
- No out-of-pocket costs for single-only benefits!
JOB SUMMARY:
- Coordinate hospice care by working with other team members as well as with the patients/families to ensure needs are met
- Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis
- Perform evaluations for hospice care along with informational visits
- Utilize the nursing process and best practices to provide professional care
- Coordinate patient care services with other members of the care team as well as contracted services when needed.
- Update and document care plans and changes in the plan of care. Consult with the attending physician and Hospice Medical Director when needed.
- Actively participate in QAPI meetings and activities as well as staff development meetings
- On-call requirement
REQUIREMENTS:
- Must be licensed RN in the State of Pennsylvania
- 1 year of Hospice experience is preferred
- Must be a licensed driver with an insured reliable vehicle
- Must have COVID vaccinations
