Housing Case Manager - Hiring Immediately
Overall Purpose of the Job
Working in partnership with Alice Griffith residents, collaborates with property management and community partners to maintain housing stability. Provides onsite referrals to services based on assessments and service plans developed with residents. Responsible for conducting outreach and engaging with residents to utilize services available to them. Assists with housing retention and community building strategies under the supervision of the Housing Director. Provides support for informational workshops, and community events as a member of the team of providers at Alice Griffith.
Availability and Hours
Monday - Friday, 8hrs/day
Major functions/ responsibilities
- Responsible for meeting goals of the overall housing stabilization program activities
- Advocates for clients at risk of losing housing by filing appeals, writing letters, arranging for subsidies, calling housing managers, attending hearings and legal proceedings
- Acts as liaison with property managers and clients. Provides information and attends weekly meeting with property management and Bayview Senior Services staff to coordinate information
- Ensures documentation is kept on all clients regarding housing stability needs via weekly housing tracker shared with Property Management and HOPE SF
- Conducts outreach and provides support for meetings, trainings and workshops that support housing stability and community engagement.
- Receives and follows up on referrals from residents themselves, property management and community partners
- Develops case plans with clear goals and services that relate to resident’s needs and inputs plans into LEARN computer system to document service needs
- Initiates referrals and works with service providers and internal agency staff to assist clients with service needs, documenting services rendered in the LEARN system and client charts in a timely manner
- Writes progress notes in a timely manner with a minimum of one note per month for active clients
- Arranges for assistance from social service agencies, involving the client in getting services needed.
- Ongoing follow up, via home visits and/or phone, providing a yearly comprehensive re-assessment for residents who need it.
- Coordinates services with other internal agency departments so that clients access other Bayview Senior Services programs to ensure maximum integration of services
- Represents Bayview Senior Services at community meetings as assigned
- Other duties as assigned
Educational/ Other Requirements
- At least two years of experience in a community working in the housing management field, preferably with low-income housing residents.
- Knowledge of community housing resources and how to access them
- Ability to relate to and speak honestly with housing residents, maintain confidentiality and maintain respectful communication with clients
- Can creatively problem-solve, communicate and coordinate with community partners
- Good organizational, written and computer skills to keep data current
- Ability to advocate for, as well as empower clients
