Job Title: Intake Coordinator – Certified Community Behavioral Health Center (CCBHC) Grant

Reports to: CCBHC Project Coordinator/Data Collector

Job Purpose: The Intake Coordinator is responsible for providing clinical, recovery support services, and case management services to all newly established patients with the Certified Community Behavioral Health Centers (CCBHC) Grant program through the Substance Abuse and Mental Health Services Administration (SAMHSA). This position is responsive to the needs of adults and children with a Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), Substance Use Disorders (SUD), and co-occurring substance use (COD) using a patient-centered approach to holistic, wraparound services. This position will provide whole-person coordination of care across medical, behavioral, and social service community systems

Essential Duties and Responsibilities:

  • Provide clinical support and case management services to the patient by starting intake process by gathering information collaboratively with clients and entering information into the electronic medical record.
  • Develop, implement, support, and promote health service strategies, tactics, policies, and programs that drive the delivery of quality healthcare to our patients by assisting in establishing medical and mental health homes. Refer to necessary clinical supports such as therapy, psychiatry, etc.
  • Meet with all new patients to obtain insurance information, assess for sliding fee scale eligibility, complete and have patient sign appropriate releases and verify insurance information.
  • Administer social determinants health screen scale to determine appropriate next level care.
  • Facilitate appropriate physical, behavioral health, psychosocial wrap around services.
  • Participate and assist in emergency and crisis services. Assist in problem solving and crisis intervention to patients.
  • Responsible for CCBHC patients involved with grant to collect and report two types of GPRA data: Infrastructure, Prevention and Promotion (IPP), and National Outcome Measures (NOMS).
  • Provide face-to-face interviews with new patients at intake to services and every six months until the completion of service provision at which time a discharge interview should be conducted.
  • Provide assistance to existing patients to complete and update financial/insurance information. Review financial and insurance options with patients who are uninsured or underinsured.
  • Assist patients with applying for Presumptive Eligibility for Medicaid when appropriate.
  • Educate new patients on SMHC services and policies.
  • Make internal and external referrals as needed.
  • Enter patient insurance information into the electronic health record.
  • Electronically scan and file all incoming and outgoing documents and medical records into the patient charts under the appropriate category.
  • As needed, back-up support to Medical Records and other support staff.
  • Conduct in-depth interviews/assessments to gather complex information from patients.
  • Demonstrate excellent customer service and collaborate with physicians, consumers, members, visitors, staff, and the health care community.
  • Assist patients in finding, accessing, and maintaining eligible benefits and resources within the community.
  • Demonstrate flexibility in job duties and schedule in order to aid in better serving the organization and patients. Allow for flexibility in scheduling to accommodate crisis situations for patients.

Experience and Education:

Required:

  • Knowledge and experience working with mental illness
  • Previous customer service experience
  • Previous insurance experience
  • Strong organizational and detail-oriented skills; ability to work independently
  • Strong interpersonal skills and be able to work positively with a wide range of people

Preferred:

  • Knowledge of community resources
  • Bachelor’s degree in a human service related field or have attained the requisite skills from prior work experience (2-3 years in a patient benefits or health insurance support capacity)
