LADC Assessor – Hennepin County Jail

Job Description

MAJOR RESPONSBILITY: To provide in-person assessment services at the Hennepin County Jail. Services to be provided with a client-centered ideology and within CREATE, Inc. mandated timelines; as means of ensuring clinical service compliance, outcomes, and client safety.

JOB DUTIES:

· Implement clinical services as directed by CREATE, Inc. clinical service policies, relevant agency policies, and CREATE, Inc. Programs Director.

· Provide clinical services which are client-centered, cost effective, and in accordance with all agency goals/objectives – within designated reporting periods.

· Attend weekly clinical supervision as directed by Site Supervisor.

· Utilize evidence-based practices as directed by Programs Director.

· Consistently maintain client records which are HIPAA compliant, meet funder requirements and are audit ready.

· Provide back up for direct care services when directed by Site Supervisor.

· Dutifully follow all CREATE systems in regard to the safety and welfare of clinical service clients.

· Build and maintain community relationships that will enhance clinical service outcomes, client referrals and client care.

· Provide and report clinical service information to the Site Supervisor.

EDUCATION AND EXPERIENCE:

· BA in Human Service associated, or related field

· Current/valid MN BBHT licensure – LADC

· Minimum 2 years experience in delivery of clinical services in 245g setting

QUALIFICATIONS:

· Superior skills in client centered delivery of services

· Superior communication and interpersonal skills

· Demonstrated knowledge of evidence based clinical service tools in a substance use disorder model of service delivery

· Demonstrated knowledge of working with a diverse population

· Demonstrated knowledge of a trauma informed delivery of services

· Demonstrated knowledge of EHR (Procentive)

· Demonstrated practice of high ethical standards

Job Type: Part-time

Pay: $30.00 per hour
