Job Description

Primary responsibilities of this SNF Nurse position is to provide care planning, resident assessment, medication administration and to monitor activities performed by certified nursing assistants. The Trinity Care Center is a 90 bed facility in a community that enjoys charm of a historic MN city along with vibrant, new development. 12hr, 8hr or 6hr shifts with flexible schedules are available.

Qualifications and Skills

Job Summary:

The primary purpose of this position is to provide direct nursing care to individuals receiving services and to monitor the day-to-day nursing activities performed by nursing assistants to ensure that the highest degree of quality care is maintained at all times.

Job Requirements:

• Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse in Minnesota.
• Must possess current Cardiopulmonary Resuscitation (CPR) certification.
