Immediate full time opening for a LPN/Care Manager at our Crossville campus.
The intent of this position description is to provide a representative summary of the major duties and responsibilities performed by the employee classification. Employees may be required to perform job-related tasks other than those specifically presented in this description.
Responsibilities fall into two general categories. The categories include but are not limited to:
Integrated Care functions
- Liaison with outside providers, including but not limited to physicians, laboratories, pharmacies, etc.
- Serve as support for an integrated care environment by working collaboratively between behavioral health and primary care to support the functions of the clinic.
- Medical Scheduling with reminder calls
1. Comprehensive Care Management – Initiate, complete, update, and monitor the progress of a comprehensive person-centered care plan (as needed).
2. Care Coordination – Participate in the patient’s physical health treatment plan as developed by their primary care provider as necessary. Support scheduling and reduce barriers to adherence for medical and behavioral health appointments. Proactive outreach and follow up with primary care and behavioral health providers.
3. Referral to Social Supports – Identify and facilitate access to community supports (food, shelter, clothing, employment, legal, entitlements and all other resources). Communicate patient needs to community partners. Provide information and assistance in accessing services.
4. Patient and Family Support – Provide high-touch in-person support to ensure treatment and medication adherence. Provide caregiver counseling and training. Identify resources to assist individuals and family supporters.
5. Transitional Care – Provide additional high touch support in crisis situations. Participate in development of discharge plan for each hospitalization. Develop a systemic protocol to assure timely access to follow-up care post discharge. Establish relationships with other treatment settings. Communicate and provide education.
6. Health Promotion – Education the patient and his/her family on independent living skills with attainable increasingly aspirational goals.
Medical functions
- Perform routine medical tests
- Provides prescribed medical treatment
- Takes and records patients vital signs
- Observes patients and reports adverse reactions to mediation or treatment to the medical personnel in charge
- Administers specific medication as prescribed by a medical provider and notes time and amount on patient’s charts, including injections
- Collects samples, such as urine, blood, sputum, from patients and performs routine laboratory tests on samples
- Proper disposal of bio-hazard materials
- Gather and enter assessment information into patient’s medical record
- Provides patient care in a non-judgmental, non-discriminatory manner that considers cultural diversity and age appropriateness so that autonomy, rights and dignity are preserved
- Handle medication phone calls
- Other duties as assigned
EDUCATION / EXPERIENCE:
Must have a Bachelor’s degree in a health-related field of counseling, psychology, social work or other behavioral sciences. Completion of a state-approved certified nursing program and the successful passing of a competency program which meets state regulations.
Must possess an unrestricted certification as a LPN.
Must have at least 6 month work experience as a LPN in a healthcare/psychiatric setting.
Licensed Practical Nurse with Tennessee license. Experience working with psychiatric clients preferred.
Must possess skills/experience in an office setting. Must be computer literate, able to complete multiple tasks, and work as a team member.
