LTC Services Screener I
Screens requests for long term care services regarding applicant eligibility needs.
Processes Community Care for the Aged and Disabled (CCAD) referrals for unit
assignments. Provides general and specific information about Community Care
Services Eligibility services to individuals and the public. Makes referrals to other
program areas and to community organizations.

Essential Job Functions:
Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
Communicates on a basic level with others (internally or externally) to provide, exchange, or verify information, answer inquiries, address issues, or resolve/report problems or complaints.
Operates computer to document, update, or inquire into case records.
Interviews individuals to assess potential eligibility for Health and Human Services programs.
Serves as an advocate for individuals and agency representative in locating available resources and making referrals to community resources when appropriate.
Knowledge Skills Abilities:
Knowledge of Gerontology, the field of disability, the dynamics of the aging process, and the special problems of the aged and disabled.
Knowledge of business English.
Education which demonstrates the possession of knowledge, skills, and abilities for job performance.
Ability to work effectively with employees and the public.
Knowledge of spelling, punctuation, grammar.
Ability to understand and carry out moderately complex oral and written instructions.
Ability to make minor decisions using rules and regulations as they apply to work problems
Ability to maintain confidentiality.
Ability to effectively communicate orally and in writing.
Ability to set priorities, establish timeframes, and meet deadlines.
Ability to establish and maintain effective relationships with individuals receiving services, co-workers, contract agency staff, and staff from other federal or state agencies.
Ability to operate computer and general office equipment.
Knowledge of interviewing techniques to obtain personal information, to make inquiries, and to resolve conflicting statements.
Knowledge of Gerontology, the field of disability, the dynamics of the aging process, and the special problems of the aged and disabled.

Initial Selection Criteria:
Education equivalent to a high school degree.
Applicant must have a minimum of two years’ experience using Microsoft Word and Excel.
Must have experience with customer service, public communication and effective organizing, tracking and working with electronic documents.
At least two years of professional experience in telephone etiquette or interviewing and accurate data entry.
Experience with database software and general computer knowledge.
Experience in organizing, prioritizing, multi-tasking and managing time efficiently.
