Manager of Clinical Operations - Eddy Senior Care Rotterdam/Schenectady NY

Eddy Senior Care PACE is looking for a dynamic leader who holds a valid RN license to join out team!

As a manager your overall responsibility will include:

• Supervision of home health aides and LPNs.
• Making participant and personnel assignments, coordinating participant care, assurance participant care needs are assessed and assuring participant individualized plans of care are monitored.
• Participates in planning, implementation, and evaluation of programs and ensuring quality participant care and outcomes, adequate/ qualified clinical staff, regulatory compliance and support achieving Agency mission, plans, goals, and budget.
• Acts as an educator and resource on community health nursing for staff, clients, and the community.

Work Schedule:

• Monday Through Friday (8- 4pm)
• On call rotation every 7 weeks

Eddy SeniorCare is an exciting, innovative model of care for seniors,_ located at 1938 Curry Road in Rotterdam_. Our Program of All-Inclusive Care for the Elderly provides a team-based model of care including physicians, RNs, LPNs, Home Health Aides, physical/occupation/speech therapists, social workers, transportation, a day center team, and more! We care for patients in the surrounding community. Collaborates closely with the interdisciplinary care team. Participates in the on call rotation every 7 weeks.

Requirements:

• NYS RN Licensure. Experience in home health preferred but not required.
• Must possess a passion for caring for seniors and demonstrate a respectful, caring, professional demeanor.
