MDS Coordinator Job at The Estates of Hyde Park
Estates of Hyde Park is looking for an exceptional RN or LPN MDS Coordinator.
MDS experience is required.
The LPN/RN MDS Coordinator coordinates and assists with completion and submission of accurate and timely interdisciplinary MDS Assessments, CAAs, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, regulations, and Estates of Hyde Park Nursing & Rehab standards.
Education, Experience, and Licensure Requirements:
- Must possess an associate’s or bachelor’s degree in nursing from an accredited college or university
- Must maintain an active RN or LPN License in Illinois and in good standing throughout employment.
- Must have one (1) year nursing experience. Geriatric nursing experience preferred.
- Must have CRN C Certification (clinical compliance)
