The Rochester Presbyterian Home is seeking a Full Time Nurse Care Manager for our Homestead Community located on Thurston Road in Rochester.

If you are an experienced elder care LPN/RN who is goal oriented and inspires a person-centered approach to care, you may be the perfect candidate to join our family.

The primary purpose of this position is to coordinate personal care services for elders in accordance with federal, state, and local standards and regulations. The Nurse Care Manager leads the care team to ensure elders are treated with respect and dignity and can pursue a meaningful way of life.

Job duties and responsibilities will include, but are not limited to:

• Evaluate and advocate elders’ medical needs and changes in partnership with the medical team and outside services.
• Responsible for medication management and distribution.
• Provide continuous input for individual Elder wellness plans that addresses health and well-being.
• Lead team in taking care of Elders, assisting with meals and ADLs.
• Assist in planning and implementing daily engagement with community life team.

Qualifications

• Licensed NYS RN or LPN
• Excellent clinical skills including person directed care approach
• Understanding of the Eden Alternative philosophy preferred
• Experience in working in a memory care environment
• Excellent customer service in collaborating with elders, families and staff
• Management experience preferred
