Nutritionist (RD)/ PRN is an important position in home health care, and we are currently seeking a qualified individual to join the Lucent Health team. Someone who has experience, knowledge, and leadership skills, who is dedicated and passionate about their career and the communities we serve, and who is looking for growth and advancement.

Job Responsibilities:

• Direct, supervise, evaluate, and provide nutritional services, on an intermittent basis, to patients, in their homes, in accordance with the physician's orders.
• Assess and evaluate the nutritional status of patients.
• Assist the physician and nursing personnel to determine the dietary needs of the patient.
• Prepare and calculate a therapeutic diet to meet specific requirements of the patient and confers with the physician to establish dietary treatment.
• Instructs on dietary restrictions and drug/nutrient interactions, as appropriate.
• Observe and report to the physician and nurse supervisor the patient's response to dietary treatment and any related changes in the patient's condition and revise nutritional plan as indicated by patient's condition.
• Communicate with other team members and, when appropriate, instruct them in nutritional, dietary teaching that they may use while working with the patient.
• Maintain and submit written clinical and agency records, as deemed necessary.
• Perform additional duties assigned in response to patient and/or Agency needs.

Job Conditions

• Must have a driver’s license and be willing, and able, to drive to patients’ residences.
• The ability to access patients’ homes, which may not be routinely wheelchair accessible, is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.
• Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients.
• The ability to communicate, both, verbally, and in writing as frequent communication, by telephone in English, is required.

Qualifications

• Must be a college graduate.
• Must be licensed or registered in the state of Florida.
• Must have a minimum of 1 years’ experience as a Registered Dietitian.
• Must have a criminal background check.
• Must have a current CPR certification.
