The Family Advocate is primarily responsible for the intake and assessment of families into the Family Services program, documenting each case appropriately and providing appropriate supportive services to meet each family. The Family Advocate will work with families to assist them to transition into more permanent housing arrangements. The Family Advocate will also develop community partnerships and coordinate with other organizations and agencies, to develop a well-rounded referral base and provide necessary follow-up to ensure the required services have been received..
Duties and Responsibilities
1) Case Management
a) Carry a caseload of a minimum of 20-35 families to aid the transition into permanent housing
b) Conduct bio-psychosocial assessments with each family to determine the current and ongoing needs of the family
c) Utilize a variety of interventions to help the family achieve their goals as it relates to housing, education and job training, employment, health care and other needs identified.
d) Provide ongoing support to each family enrolled in the program and aftercare caseload.
e) Work with clients in establishing short and long term goals and objectives through the utilization of interventions.
f) Provide conflict resolution, problem solving, planning and life skills development support to all clients on your caseload.
g) Meet with clients regularly to ascertain progress and determine additional needs
h) Adhere to strict boundaries and professional ethics in the care of others
i) Coordinate and facilitate group events, including parent, community and psycho-educational meetings
j) Ensure effective service delivery by coordinating work with other case managers from other agencies serving the client.
k) Work with other Family Services staff to consistently update information and contacts with other community organizations providing services and resources to the families
l) Build positive working relationships with other community organizations and resources.
m) Provide clients with appropriate community referrals, and follow-up to assess the effectiveness of these referrals.
n) With the other Family Services staff, evaluate need for program changes and additions, and under the direction of the Family Services Director, work to implement agreed upon changes/growth.
o) Participate in interagency and external collaborative efforts and community groups as requested by your supervisor.
2) Housing Assistance, Referrals, Groups
a) Develop case plans with each family using empirical goals and measureable outcomes
b) Work closely with the client’s external housing case manager and visit housing placement if necessary and appropriate.
c) Work with clients to assess barriers to housing and refer clients to appropriate community resources as needed
d) Assists clients with housing applications, government and health care assistance applications and other applications for resources
e) Coordinate transition to Permanent Housing
f) Partner with EHS to complete recruitment and enrollment into ECE program
g) Partner with other Family Advocates to provide support and educational groups
3) Maintaining Client Records and Data Entry
a. Complete intakes and exits with families for program enrollment and discharge
b. Maintain accurate documentation of service objectives, interventions and outcomes as well as other services in accordance with Federal, State, County and other guidelines.
a) Maintain and enter data into software systems Pathways, Child Plus, Apricot and other internal or required data collection systems
b) Assist with the compilation and tracking of grant delivery and report deliverables
c. Assist with grant reporting requirements as needed and assist with data “clean-up” and tracking
4) Conduct other duties as assigned
Qualifications
Education: Bachelor’s degree in a social science or related field required, Master’s degree in social work, psychology, counseling or related field preferred.
Experience: 2 years related experience required, prefer one year working with homeless families
Skills: Interpersonal skills, especially the ability to work well with children and families
Management, organizational and planning skills, multi-tasking independently with little supervision
Excellent communication skills, both oral and written
Interpersonal skills, especially relating to volunteers from diverse backgrounds and experiences
Computer skills including proficiency in Microsoft Office suites and the ability to catch on fast with semi-complicated and complex software systems.
Job Type: Full-time
Pay: $40,000.00 - $45,000.00 per year
