Essential Duties & Responsibilities
- Works with client to learn or regain skills for successful community living including independent living skills (shopping, cooking, cleaning, budgeting and transportation) and socialization (enhancing social and/or romantic relationships, recreational and leisure pursuits that contribute to community integration) utilizing deliberate and consistent skills training which typically includes staff demonstration, working side by side, facilitating groups, client practice/roleplaying and staff feedback, as well as prompting and cueing
- Works directly with the client to help them identify their goals, find resources to help them grow, and help their clients take necessary steps to reach their goals.
- Works with clients to understand their needs and support them as they work to build a more fulfilling life for themselves.
- Drive the client to doctor's appointments, library, run errands and other places that client needs to go.
- Develop a comprehensive social history and psychosocial assessment that includes the client’s problems, strengths, and preferences; including cultural, religious, and ethnic background and implications for the treatment plan.
- Communicate with the client's family in regards to client's progress towards achieving goals
- Attends scheduled case conferences and identifies and interprets psychosocial needs of consumers for inclusion in the case plan.
- Completes required clinical documentation within timelines established by applicable regulations and policies.
- Ensure the timely completion of initial and concurrent Individualized Treatment Plans in collaboration with the consumer and identified supportive persons.

Compensation & work hours
- Part time: $20/Hour
- Monday - Friday: 10am-2pm
