POSITION SUMMARY:
As an integral member of the pediatric primary care team the Pediatric Care Management RN will have the opportunity to make a profound impact on the lives of pediatric patients and their caregivers living with complex and/ or chronic conditions, many of whom also face multiple barriers accessing care and need support to succeed with achieving health care goals. This position requires flexibility and may vary from day-to-day to meet patients and their caregivers where they are. Outreach methods may vary based on the needs of the organization and may include telephonic or in person in a variety of potential settings such as but not limited to, the community, home, facility, or health center.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Maintains a professional environment in a multi-provider, multidiscipline organization.
Maintains a patient centered environment including but not limited to:
Provision of care and advocate for pediatric patients under 22 years and their families based on prudent nursing judgement and acceptable standards of practice.
Performs tasks and procedures within the scope of a registered nurse (R.N.) and legal guidelines of the nurse practice act.
Works closely with the primary care provider in an effort to decrease inappropriate use of the emergency room and unnecessary hospitalizations. This includes medical, mental health, or substance abuse issues.
- Works to increase access to primary care and care coordination through a multidisciplinary approach.
- Participates in improving medical management services to patients with chronic and/or difficult to manage diseases.
- Participates in improving efficiency and effectiveness of patient services.
- Works with the case management team to achieve goals of the case management program.
- Patients in need of management of care will be given an appointment with the case management registered nurse.
- Patients are to be educated regarding their disease condition and proper care. Develop plan of care.
- Assess patient for services needed by other disciplines and arrange an appointment.
- Review medication(s) indication and side effects.
- Supervise and coordinate care with Pediatric patient navigator
Other Patient Services
- Prescription refills and DME paperwork as needed
- Telephone Triage and follow up calls for care management patients
- In person visits and assessment
- Nursing Assessment including vital signs, Fingerstick blood sugar, peak flows
- Documentation of care plans and assessments

QUALIFICATIONS
QUALIFICATIONS
- Bachelor of Science Degree in Nursing preferred
- Graduate of an accredited school of nursing
- Current state licensure to practice in Massachusetts.
- Cardio-pulmonary resuscitation (CPR) certified
- Maintains licensure and certifications.
- A minimum of five years of nursing experience at least 2 years in Pediatrics .
- Bilingual preferred.
MINIMAL KNOWLEDGE BASE:
- Demonstrates knowledge of:
- Nursing principles including disease processes, certain examinations, observations, laboratory studies and appropriately applies these principles to patient care.
- Managed Care.
- Need computer skills. Must be able to learn and adapt to new computer programs.

- Maintains and encourages teamwork approach.
- Flexibility – continuously participates in the modification of schedule, duties, and procedural functions.
- Contribution – consistent effort to further goals by modifying and/or improving individual procedures and tasks.

- Identifies tasks and operational problems. Generates and implements solutions in conjunction with co-workers and management.
- Maintains and encourages good communication skills.
- Participates in orientation and training of new staff
WORKING CONDITIONS:
Works in a busy community health center in a downtown business district. Long hours are sometimes required. Some in-state travel may be required.
PHYSICAL REQUIREMENTS:
Physical demands requiring lifting of small pieces of equipment, boxes, and/or patients not to exceed forty (40) pounds. Mostly standing and walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversation with or without audio devices.
