CHC CRS is the only multi-specialty interdisciplinary clinic (MSIC) in Northern Arizona. Our 20,000 square foot clinic is designed to provide the best patient outcomes possible. Our services include 12 different pediatric specialty services, physical therapy, occupational therapy, speech therapy, registered dietician, social work, audiology and full case management. In addition, we have several other services to assist with wheelchair, medical equipment and orthotics evaluation and dispensing, hearing aids, feeding therapy and dental screenings in partnership with Northern Arizona University.
CHC has additional specialties to better serve our community. NAH-Inspire is a lifestyle treatment program specializing in treating pediatric obesity. And, our Safe Child advocacy program dedicated to provide comprehensive child abuse intervention services to children and families who have been victims of crim. The role of RN Case Manager includes assessment, planning, implementation, education, communication and evaluation of medical and therapeutic services to address the needs of patients and their families in order to achieve desired outcomes. The RN Case Manager is expected to utilize the nursing process and practice at the top of their license in accordance with the Arizona State Board of Nursing Laws and Regulations. Responsibilities:

Care Management
- Coordinates the appropriate healthcare resources for the patient and their family.
- Initiate consultations with other health care professionals to promote an interdisciplinary plan of care for complex situations.
- Serves as a liaison for the patient, family, care team and ancillary services.
- Make necessary referrals to appropriate members of the healthcare team and/or outside agencies.
Documentation
- Obtains and archives relevant patient data in medical record.
- Revises and updates plan of care as the patient's parameters and needs change.
- Appropriately documents information in patient medical record.
- Maintains timely and accurate documentation of assessments, interventions, EMR entries, and patient responses.
Education
- Provides patient and their family with education regarding management of health care needs.
- Acts as a resource to other departments and the community. This includes fielding questions, mentoring, and/or providing educational opportunities.
Patient Care
- Provides patient and their family with assistance to resolve any conflicts or issues they encounter while navigating their healthcare needs.
- Supports patient and their family in planning for discharge.
- Communicates information clearly and in a manner appropriate to the level of patient and family understanding.
- Facilitates the implementation of individualized treatment plan as directed by the patient's care team.
Compliance/Safety
- Responsible for reporting any safety-related incident in a timely fashion through the Midas/RDE tool; attends all safety-related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.
- Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.
- If required for the position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.
- Completes all company mandatory modules and required job-specific training in the specified time frame.
- Completes charges in a timely manner and according to proper procedures.
- Responds to new orders promptly in compliance with department policy.
- Adheres to regulatory and organizational standards affecting patient care and safety as well as all hospital and departmental policy and procedures.
