Description:
We are seeking a skilled Physical Therapist for a field position in the Rocky Hill area. As a member of our team, you will provide exceptional care to patients in their homes, helping them recover from injuries and illnesses.
Qualifications:
- 2 years of experience in specialty
- Candidates may be required to float to another branch—but only if discussed at the time of offer
- Locals accepted
- Required: Home Care Home Base, Oasis
- Must have Previous Adult Home Health Experience
- Will consider temp to hire
- Milage Reimbursement IRS Standard
- Must meet 30 productivity points
- OT preapproved by the branch
- No on-call unless stated upfront
- COVID-19 Vaccine (Facility Guideline): Required - Medical/Religious Exemptions Only
Responsibilities:
- Perform comprehensive assessments and develop individualized treatment plans for each patient
- Provide hands-on therapy to patients in their homes
- Communicate regularly with physicians, nurses, and other healthcare professionals to coordinate care
- Document patient progress and outcomes in an accurate and timely manner
- Educate patients and their families on home exercise programs and other self-care strategies
- Maintain compliance with all state and federal regulations
