Top 10 Diagnosis: Members with multiple comorbidites and with potential social barriers, leading to health issues. The demand for emergent and acute care is often driven by social determinants outside the scope of the medical model. So familiar with community resources to be able to link members. Will Float to Modesto-Manteca-Stockton Age Groups of Patients: Peds, Adolescent, Adults, Geriatric Professional dress code Common Disciplines: Care Without Delay Director, Contg Care Util Rvw Coord RN, Social Svcs Clin Specialist & Pharmacist

CA-PT, outpatient experience, BLS

This recently built, full-service hospital provides outpatient services, an on-site pharmacy, and more than 100 physicians across 50 departments.
