Job Responsibilities:
· The Prevention HUB Specialist responds to inquiries for information about substance prevention, early intervention, treatment, and recovery support services as well as other services needed.
· The Prevention HUB Specialist will determine caller needs to identify appropriate providers that offer support and resources.
· Empowers the individual to self-refer to local services. When appropriate make referrals on the individual's behalf, with the individual's permission, to local service.
· The Prevention HUB Specialist provides a warm hand-off when appropriate, to services within and outside of the agency.
· Ensures relevant documentation is completed to facilitate referral process.
· Establish and strengthen community partnerships by attending resource-sharing meetings, health fairs and other community events, as assigned.
· The Prevention HUB Specialist promotes the use of assessment tools to service providers to determine qualifying mental health and substance use disorder treatment needs.
· Build out a database of local service providers in collaboration with ADAPT staff, Family Connection staff, and community partners.
· Surveys individuals who utilize HUB resources/referrals to collect satisfaction feedback with services provided.
· The Prevention HUB Specialist must assist with the coordination of additional coalition program functions, including needs assessment, facilitating various ADAPT Committees, and maintaining coalition member database as directed by the ADAPT Senior Manager and/or ADAPT Coordinator.
· Attend any required trainings both within the agency and externally, in-State and out of State.
· Assist with other duties as directed by ADAPT Senior Manager and/or ADAPT Coordinator.
Qualifications:
Education/Certification: Minimum of a BA in Public Health, Community Health, or a related field.
Experience: Previous experience with demonstrated success in facilitating groups, community organizing, and team building. Proven ability to work effectively on a team as well as strong Cultural Competence skills.
Knowledge: Knowledge of implementing evidence-based programming with the older adult and college-aged populations. Knowledge of the effects and statistics regarding pain management, underage drinking, and substance abuse and/or misuse. Ability to complete reports, collect/analyze data, and develop surveys as well as have knowledge of the tools needed to complete these measures.
Skills and Abilities: Strong clerical, organizational, computer, written and oral communication skills. The willingness to work with individuals from diverse backgrounds. The ability to work on a multi-disciplinary team.
Work Environment/Physical Demands: The individual must possess the ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, and lifting of up to or in excess of 50 pounds.
