About Novum Behavioral Health:

Novum Behavioral Health provides services to meet the emergent and ongoing psychosocial and psychiatric needs within the evolving health field. NBH collaborates with a core of high-performing providers who align with our specific model of care to offer the services that meet and exceed our member’s needs efficiently and effectively.

Position Purpose: Serve as a liaison for primary care physicians and core outpatient behavioral health providers in order to enhance coordination of care, improve clinical quality standards and promote the integration of physical and behavioral health.

Job Description:

Work with our core outpatient provider network and primary care groups to enhance coordination between behavioral health and primary care, conduct provider education and outreach, identify gaps in care, improve access to behavioral health services, establish standardized coordination processes for primary care physicians and behavioral health providers.

Create integrated care plans that include primary care and behavioral health treatment plans and SDOH needs to ensure providers responsible for care have a whole person view of the member.

Implement and manage procedures for tracking, identifying and solving problems for our core provider network.

Collaborate with staff to identify internal and external opportunities to increase quality and improve provider and member satisfaction.

Serve as a resource and liaison for provider groups regarding the following: access to behavioral health care, utilization, quality improvement and care coordination services.

Education/Experience:
Bachelor’s Degree in Behavioral Health. At least one year of experience in managed care. At least one year of experience in a community mental health and/or integrated healthcare setting.

Pay: $25.00 - $27.00 per year
