Resources for Human Development is seeking mission-driven individuals to join our team. RHD New Foundations&YALE program is currently hiring 2 Psychiatric Rehabilitation Facilitators. The Psychiatric Rehabilitation Facilitator provides supportive services to individuals with mental illness living within the Yale program and Positive Resolutions.
Position A- Wed, Fri - 7AM TO 7PM & Thurs, Sat - 7AM TO 3PM
Position B- Wed, Sat - 3PM TO 11PM & Thurs, Fri - 11AM TO 11PM
Position C- Wed -12PM TO 8PM, Thurs, Sat - 8:30AM TO 8:30PM, Fri - 8:30AM TO 4:30PM

Responsibilities/Duties
- Document progress towards recovery goals and any other significant resident interaction
- Complete daily checks of household/rooms notifying Psychiatric Rehabilitation Specialist of any safety issues or repair needs
- Teach residents how to meal plan, food inventory, and create a grocery shopping list
- Provide Psychiatric Rehabilitation Services to members of the YALE/New Foundations community
- Show residents how to make all medical and psychiatric appointments
- Collaborate with residents to identify daily living skills they would like to learn and create a plan for accomplishing those goals
- Attend and/or facilitate educational groups and meetings
- Complete required documentation including billable encounters, individual recovery plans and non-billable encounters.
- Complete intake and discharge documentation
- Maintain caseload and reqired number of billable encounters
- Provide coverage for site as needed
Required Experience
Two years of experience working with adults with Mental Health diagnoses.Required Education
- Bachelor's Degree in the Human Service field or related field preferred
- High School Diploma/ GED
Required Qualifications
- Valid Drivers License
- Ability to pass to background check
Program Summary
New Foundations and YALE is comprised of group homes for adults with mental illness. The program is located in the community and provides supervision, training, and guidance in daily living skills. Programming and service delivery is guided by the principles of recovery to assist individuals to achieve life goals. The Young Adult Learning Environment(YALE) provides additional educational, vocational, and social supports to better meet the needs of younger individuals18 to 22 years old
