Pay range for new hires: $33.80 - $54.08/hr

Job Overview:
The Whitcomb Terrace Clinical Resource Coordinator provides hands-on and administrative clinical support to Whitcomb Terrace director and staff, is a care provider and coordinator, and a change agent and advocate for the clinical needs of Whitcomb Terrace (WT). This position provides professional care in accordance with known/accepted standards of practice and WT/AVH policy, and sets the example for staff to follow in clinical practices. This position works with the director, medical director, and staff to identify expected and possible resident clinical needs, outcomes, and interventions to achieve those outcomes, working in collaboration with the multi-disciplinary team, including family members.
Primary Scope of Responsibilities:
Provides input to the Director on resident care needs; assists with policy and procedure development/review and/or revision in relation to clinical needs; and assists with and facilitates with day to day resident care needs; and assists with clinical quality improvement activities in the department.
Partners with the Director and Medical Director, and works within state guidelines, to elevate the clinical knowledge and awareness of the team, and promotes efficient and effective clinical practice patterns.
Works with the Director and Medical Director to create, implement, and maintain clinical staff education to provide formal and informal education, and serves as a mentor for the staff in clinical care needs.
Works with Director and Medical Director to facilitate and coordinate clinical care provided by the healthcare team in all areas, utilizing evidence based practice. Communicates and collaborates with other members of the team to promote best practices.
Collaborates with the team to create and maintain residents’ plan of care needs and takes an active role in care coordination.
Assists the staff to provide individualized resident and family education consistent with personal and specific resident clinical care needs.
Evaluates the effectiveness of care practices and assists the director, medical director, and team with changing methodology, as needed.
Adheres reliably to all accepted resident care and safety standards.
Identifies and promotes new learning around changing clinical care practices as needed.
Acts as a leader in clinical care awareness, and practices within scope, professional expectations, and accepted assisted living practices and standards.
Able to model and promote assisted living care standards of person-centered care for residents.
Acts as an educator to others through training and development in clinical procedures.
Demonstrates responsibility and accountability for own professional decision making.
Assists director, medical director, and other staff to solve problems relating to clinical care and needs in the department.
Assists in maintaining positive employee relations, and safe and efficient resident care. Demonstrates a commitment to a healthy work environment focused on professionalism and excellence.
Fully competent in the primary role; performs duties of personal care provider as needed.
Specific Duties:
Assists director and medical director in pre-admission assessment processes and procedures to determine applicants’ appropriateness for admission.
Assists director, medical director, families, and staff regarding clinical needs and procedures in relation to admission, transfer, and discharge.
Creates, updates, audits, and maintains Medication Administration Records (MARs) for all residents.
Works with medical director and director regarding staff training, awareness, and needs in clinical areas.
Maintains and audits Standing Orders book for current medications.
Works with pharmacies to order residents’ medications weekly and as needed per physician’s order.
Works with medical director and other physicians to complete bi-annual (and as needed) resident visits, reviews, and assessments.
Works with medical director regarding new medications, as well as medication changes. Completes necessary transcriptions on the MAR, and obtains needed signatures in accordance with procedure, updates medication count sheet.
Creates, updates, and maintains appropriate care plans for all residents. Performs ongoing resident assessments and problem identification, plans care accordingly; implements and evaluates plan of care so that problems are resolved.
Works with director and medical director to maintain protocols and practices regarding COVID-19 procedures, processes, policies, testing, results, and information.
Assists with policy and procedure development/review and/or revision in relation to clinical needs.
Minimum Qualifications:
Ability to concentrate and show attention to detail
Relatively high degree of analytical abilities
Strong interpersonal skills required
Ability to work independently
Proficiency in Microsoft word and excel
Education/License/Certification Requirements:
Current Colorado RN license.
Current certification in First Aid/CPR/AED, per state requirements
