Job Description


We are seeking a motivated individual with excellent communication skills to join the Referral Relations team at New Hope Ranch. It is the role of the Care Coordinator to monitor and ensure communication and collaboration between our onsite employees, referring professionals and our clinical outreach team regarding client care. The CC audits patients’ EMR (KIPU) charts to assist with communication and documentation during patient's treatment. Furthermore, it is the CC's purpose to effectively collaborate with EAPs and referral partners, while supporting patients in treatment to ensure appropriate referral options upon their discharge. Upon discharge, the CC will update Salesforce with the referrals for each client to the providers.

Responsibilities

  • Provide administrative and communication support for our onsite referral relations team and outreach representative to ensure compliance with company expectations.
  • Complete regular internal KIPU audits and provide results to Clinical Outreach Reps and Facility Referral Relations team to ensure referring providers and family are communicated with upon Admission, Midpoint, Discharge, and any Emergent Situations during a patient's treatment episode.
  • Manage Excel spreadsheet by documenting audit results of patients’ KIPU charts and communicate any concerns regarding documentation.
  • Follow up to ensure issues found in audit and communicated were addressed.
  • Identify patient needs for FMLA and short term disability
  • Knowledge and ability to work with designated software platforms (KIPU, Salesforce, etc.)
  • Assist with identifying appropriate resources for client’s aftercare plan when needed
  • Documenting referral options provided in appropriate provider accounts in Salesforce
  • Ability to problem solve from a solution-based place
  • Efficiently manage Employee Assistance Program and Veterans Association accounts to ensure sufficient communication with our referral partners.

  • Organized and detail oriented
  • Assists team with timely documentation of aftercare plans and outbound referrals
  • Monitors communication and collaboration with our referral partners and families while client is in treatment.
  • Submits necessary reports to Director of Referral Relations SUD

Qualifications
  • Strong Computer competence (Microsoft Word, Outlook, Excel). Must also have experience with Electronic Medical Records (KIPU), and Customer Relationship Management (CRM) platforms; (Salesforce)
  • Exemplary interpersonal skills to effectively communicate with Program Directors, Outreach Representatives and Program Clinicians.
  • Must have strong written and verbal communication skills
  • Strong data management abilities
  • Hardworking, detail-oriented and highly motivated to learn new skills
  • Previous experience working in behavioral healthcare
