Somerfield Health Center at The Heritage at Brentwood is currently seeking a full-time Registered Dietitian/Nutritionist for a temporary assignment of 16 weeks. This position assists in planning, scheduling and supervision of health center dietary personnel in developing all menus for health center residents. Somerfield at The Heritage is a 5-star, 66-bed skilled nursing facility located on the beautiful campus of The Heritage at Brentwood.

Job Responsibilities

• Applies the Nutrition Care Process (NCP), including completion of nutrition assessments, determination of nutrition diagnoses, development and implementation of evidence-based nutrition interventions, monitoring and evaluation of residents’ progress, and communicating the NCP with other members of the care team

• Utilizes Nutrition Focused Physical Exam to assess nutritional deficiencies and implement change in nutritional recommendations as needed

• Collaborates with the Medical Director, Nurse Practitioner, interdisciplinary team, wound nurse, weight technicians, and therapists to ensure resident nutrition needs and goals are thoroughly met

• Provides nutritional counseling to Somerfield residents and caretakers

• Continually monitors and updates nutritional plan of care goals to ensure goals are met and achieved

• Analyzes quality assurance and performance improvement requirements for Food & Nutrition services team and implements appropriate changes in policies and procedures as needed to maintain a 5-star rating and infection control safety practices

• Inputs and analyzes data required to complete Minimum Data Set (MDS) assessments submitted to CMS for PDPM payment model, quality assurance, and performance improvement

• Participates in interdisciplinary team and care plan meetings

Qualifications

• Must be a Registered Dietitian (RD)

• Minimum two years supervisory experience in food service operation for a healthcare setting required

• Certified in food service safety and sanitation

• Experience working with long term healthcare environment preferred

• Experience in performing clinical assessments on residents, meeting with care plan team and performing input on MDS required

• Ability to communicate with, understand, empathize with, work with and be tolerant of residents, families, visitors, peers, supervisors and subordinates

• Ability to interpret and explain menus, refer or answer questions, coordinate meal service process and understand requests

• Proficient computer skills, including Microsoft Word

• Fully vaccinated for COVID-19 or approved exemption required per CMS regulations
