Our Clinical Nutrition Team have a great opportunity for a Registered Dietitian at our Schaffer Extended Care Facility. The facility is part of the Montefiore Health System, a premier academic health system serving more than 3 million people living in New York City region and Hudson Valley. Montefiore is doing more to deliver science driven care where, when and how patients need it most offering patients state of the art healthcare close to home.

Job Duties Include:

• Provide in-depth nutritional assessments, follow-ups and education for patients at high as well as moderate and potential nutritional risk in an acute or extended care facility.
• Assess patients to determine level of care, developing and implementing nutrition care plans including discharge recommendations, which may include instructions on appropriate diet therapies to patients/family.
• Participate as a core team member in interdisciplinary coordinated patient care program and on committees for the development and revision of nutrition related policies and procedures.

Education and Requirements:

• Bachelor's Degree in a Nutrition related field required. Master's degree preferred.
• RD/RDN with the Academy of Nutrition and Dietetics
• NYS Certification (CDN) required within one year of hire
• Prior experience in an acute care/extended care facility.
