Job Summary:

The Registered Nurse is a clinical nurse with daily responsibilities in direct patient care, safety and education, oversight of Medical Assistant(s), and fostering collaborative relationships with providers and other clinical operations staff.

This position is ideal for an experienced nurse who enjoys performing daily clinical patient care, has a passion for working with the senior community, desires a day shift/Monday-Friday schedule.

ArchWell Health requires all new hires to provide proof that they are fully vaccinated from COVID-19 or represent that they will be fully vaccinated within 30 days of their start date.

Duties/Responsibilities:

  • Perform clinical activities in accordance with the policies and procedures set forth by the organization
  • Adhere to the clinical team workflows and provide direct oversight to Medical Assistants
  • Perform daily staff nursing activities; day-to-day patient activities, including phone and in-person patient triage and patient education.
  • Collaborate with the Providers and Center Manager to identify opportunities to improve clinical performance
  • Develop, implement, and monitor the effectiveness of performance improvement activities
  • Collect data pertinent to the patient’s health or the situation.
  • Analyzes the data to determine actual or potential diagnoses, problems, and issues
  • Develop a plan in collaboration with the team that helps attain expected, measurable outcomes
  • Implement the identified plan, coordinate care delivery, and employ strategies to promote health and a safe environment
  • Perform regular monitoring activities to ensure compliance with national and organization standards of care. Clinically prepares the center for internal and external quality audits and helps lead the center response to any clinical deficiencies found.
  • Maintain current knowledge of state regulations for nursing, as it relates to scope of practice for back-office personnel.
  • Identify and provide appropriate resources to educate patients, their families and staff members on clinical conditions and processes.
  • Oversee clinical quality and patient safety through training and education of the clinical team and administrative staff.
  • Communicate effectively in all areas of practice, including how to motivate and engage colleagues around a common goal.
  • Ensure that clinical processes, policies, and procedures are adhered to in the center.
  • Monitor and manage medical supplies and immunizations

Required Skills/Abilities:

· Experience with assessing patients with acute and chronic medical conditions

· Ability to plan, provide, and sustain evidence-based nursing services that are safe, effective, and fiscally responsible

· Track record of developing and implementing processes for providing excellent patient experience

· Thorough, in-depth knowledge of clinical procedures, legal rules and regulations and best practices in healthcare

· Skilled in observation, training and evaluating clinical/patient care competencies

· Must possess a high degree of emotional intelligence and integrity, driven and focused work ethic

· Continuous desire to learn and embrace new methods; ability to adapt and be resilient

· Self-starter with the ability to think creatively and work effectively

· Ability to build a relationship and work effectively with various seniorities and diverse populations including staff, providers, members, and family members

· Excellent critical reasoning, decision-making, and problem-solving skills to analyze situations, determine risks, and find solutions to prevent future issues and resolve recurring defects

· Proficiency with Electronic Medical records and Microsoft office tools.

· A passion for mentorship and team-development, particularly across multiple functional areas

· A problem-solving orientation and eagerness to identify process gaps and implement practical solutions in a matrixed organization

Education and Experience:

  • Associate or bachelor’s degree in Nursing required
  • A valid, active Registered Nurse (RN) license in state(s) of employment required
  • A minimum of 3 years of current direct nursing experience required
  • BLS certification for healthcare providers required
  • Fluency in Spanish or other languages spoken by people in the communities we serve is helpful
