Title: RN Home Health Case Manager

Location: Rochester, NY (USA)

Salary: $3200/wk

Stipend: $1281/wk

Shift Schedule: Day shift- 40hrs/wk - Mon-Fri - 8a - 4:30p

Job Type: Full-time

We are currently seeking an RN Home Health Case Manager to join our team. This position is responsible for managing patient care in the home setting, ensuring that care is delivered in a timely, efficient, and effective manner. The successful candidate will have strong assessment skills, be able to develop and implement care plans, and work collaboratively with other healthcare professionals to ensure excellent patient outcomes.

Responsibilities:

- Perform initial assessments and ongoing evaluations of patient needs

- Develop and implement individualized care plans for each patient

- Coordinate and schedule all necessary services and appointments

- Monitor patient progress and modify care plans as needed

- Educate patients and their families on disease management and self-care techniques

- Collaborate with physicians and other healthcare professionals to ensure excellent patient outcomes

- Adhere to all federal, state, and local regulations related to home health care

- Maintain accurate and complete patient records

- Participate in team meetings and training sessions as required

Requirements:

- Active RN license in the state of New York

- Minimum of 2 years of experience in home health or related field

- Excellent assessment, communication, and organizational skills

- Ability to work independently and manage multiple patients simultaneously

- Knowledge of Medicare regulations and home health care standards

- Proficiency with electronic medical records and computer software

- Valid driver's license and reliable transportation
