We are a nurse owned home care agency serving clients primarily in Wake and Northern Johnston County. We are proud to be a locally owned and operated location of Nurse Next Door.

We are looking for a Registered Nurse to fill the role of Care Designer. A Nurse Next Door Care Designer is a hybrid role consisting of leadership and supervision, administration and hands on care, education and support.

Primary responsibilities include:

  • Conduct Caring Consults (informational and intake visit), assess clients with respect to physical, emotional and environmental needs
  • Act as advisor in Caring Consult to ensure that client and client contacts have all information required
  • Complete intake paperwork and gather necessary information
  • Develop and maintain appropriate plan of care based on client needs and goals
  • Attend first visits with caregivers to ensure client needs will be met and care plans are understood
  • Conduct supervisory visits to ensure caregiver fit and ongoing quality of care
  • Update care plan as required
  • Educate clients on self-care and evaluate response to care provided
  • Lead caregiving team by setting clear goals, assigning tasks, coaching in the moment and providing continuing education
  • Manage education to support a continuous learning environment and utilization of learning opportunities. This includes Discovery Day (orientation) and monthly in-service programs
  • Responsible for data integrity and completion (caregiver & client files) in the scheduling software
  • Participate in on call rotation with other nursing and administrative staff (scheduled may vary)

Administrative duties include:

  • Maintain excellent documentation practices including complete and accurate care plans, employee files (skills & qualifications), client files, including pay and billing information
  • Review and update client and staff records to ensure accuracy
  • Conduct record audits to ensure accuracy and up to date information
  • May participate in employee selection processes including recruitment, hiring and onboarding of staff

Qualifications:

  • Active North Carolina RN license in good standing
  • Passion for working with older adults
  • Desire to be an educator and role model
  • Minimum one year experience as a Registered Nurse (preferred, will consider outstanding new grad)
  • At least one year of experience in long term care or skilled nursing preferred
  • Excellent written and verbal communication skills
  • Demonstrated critical thinking skills
  • Supervisory experience preferred
  • Proficiency with computers, including g-suite, and clinical documentation software required

Job Types: Full-time, Part-time

Pay: $40,000.00 - $70,000.00 per year
