We are seeking a personable, compassionate, and experienced full-time Registered Nurse or Licensed Practical Nurse to handle phone calls from our cardiology patients with questions that require clinical knowledge. The successful candidate will have cardiology experience (nephrology experience is also a plus) and will function as an important part of the clinical team. This position is located in our wellness and specialty clinic in Derry, NH.

This is a full-time (32 - 40 hours) position.

Responsibilities include:

• Nurse visits for insulin or other medication (injectable) teaching, EKGs, heart monitor placement education, blood pressure review, other nurse visit related education.
• Follow up with patients with CHF, COPD, Diabetes via phone call or in-person.
• Provide education to patients per provider request during an office visit.
• Manage triage calls from patients who have questions related to medications, prescriptions, referral requests, immunization needs, lab results, illness symptoms, etc.
• Serve as liaison between the provider, care management team and the patient. Excellent communication skills are required.
• Escalate calls when necessary.
• Document communications in EMR with excellent attention to detail.

Qualifications:

Graduation from an accredited school of nursing is required. Candidates must hold a current State of New Hampshire Nurse’s license. Candidate must be able to communicate effectively, both orally and in writing. A minimum of one year of acute or ambulatory care experience is required.
