Overview:
Responsible for the assessment, planning, implementation and evaluation of patient care in the behavioral health outpatient setting for partial hospitalization. Responsible for the delivery of patient care through a multidisciplinary approach. Responsible for case coordination of patients and insurance precertification/utilization review. Patients served are adult behavioral health patients requiring a structured outpatient setting. Patients to be served include adult and geriatric. St. Luke’s C.A.R.E. delivery model (Continuity, Accountability, Relationship-based, Evidence-based) reflects a commitment to compassionate, competent care delivered by the interdisciplinary team. Care is provided through a therapeutic relationship with the patient and family. A spirit of inquiry is encouraged and supported. Small tests of change and more formal research efforts are used when the evidence is not clear. Evidence-driven protocols are utilized to assure reliable and consistent care.

Responsibilities:
Clinical Excellence/Patient Care
  • Admission assessment includes completion of the comprehensive evaluation of the mental status – patient’s mood, affect, though content and psychosocial issues.
  • Initiates treatment plan, coordinates care with multidisciplinary team and admitting physician per department policy.
  • Maintains competence in performing procedures involved with the management of violent or potentially violent patients.
  • Precerts all admissions to the program with their insurance carrier, works out financial arrangements with patients without insurance coverage.
  • Coordinates cases with community agencies, referral sources.
  • Manages utilization review for own patients with insurance carriers.
  • Provides structured educational groups for partial hospitalization patients.
  • Assists other departments with assessment of patients who may need outpatient behavioral health services as requested.

Development & Patient Education
  • Continues professional development and involvement in professional activities that enhance nursing practice by sharing knowledge through networking and research.
  • Assess and chooses teaching strategies appropriate to the patient’s and their families’ age specific needs, culture, gender, readiness to learn and level of comprehension.
  • Consults appropriate resources within the hospital to provide specialized education.
  • Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.
Qualifications:
  • Graduate of accredited nursing program
  • urrent licensure in good standing to practice as a Registered Nurse in [type in State].
  • Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.
  • Writes, reads, comprehends and speaks fluent English.
  • Multicultural sensitivity.

  • Critical thinking skills using independent judgment in making decisions.
  • Use of usual and customary equipment used to perform essential functions of the position.
