The MMI Care Coordination Social Worker has the responsibilities of a clinical service provider who provides direct services in the Care Coordination Program. The position entails working within an interdisciplinary team that may include social workers, behavioral health providers, rehabilitation providers, interpreters, medical providers, and parent resource coordinators to carry out effective care coordination activities across the MMI clinical service delivery departments and programs. The position provides direct and consultative services to patients and caregivers of individuals receiving clinical services at MMI.

4. Ability to handle confidential matters discreetly.
5. Knowledge of electronic health record documentation.
6. Implementation of best practices related to the delivery of care coordination and case management
