Substance Use and Psychiatric RN/LPN Job at Hilltown Community Health Center
LINE OF RESPONSIBILITY: The substance use and psychiatric nurse reports to the Director of Nursing with support from Clinical Operations Manager
PURPOSE: This position works to provide comprehensive mental health care and substance use treatment to the under-served populations currently being managed by our homeless medicine doctor
RESPONSIBILITIES:
Identify individuals struggling with mental illness in community and clinical settings, build rapport, and work as liaison in treatment- Provide Hepatitis C disease and treatment education, assist in getting medication to patients for treatment
- Provide chronic disease management at the direction of the homeless medicine doctor, including HTN monitoring, wound care, medication education, psychiatric support
- Work closely with homeless medicine doctor to provide comprehensive health care
- Development of nurse based protocols for SUD and Hep C treatment
- Work with DON to develop and provide de-escalation training for all patient facing staff
- Provide nurse led substance use treatment visits based on protocol
- Assist in education other staff nurses on triage of complex mental health and substance use patients
- Travel to both Manna and Amherst to provide care as needed
- Work with homeless medicine doctor to provide care in the community as needed
QUALIFICATIONS:
- Proven project organization experience
- Excellent oral and written communication skills
- Must display excellent teamwork
- Ability to work in a fast -paced environment
