SUPPORT COORDINATOR--Clinton County Senate Bill 40
Clinton County Senate Bill 40 provides case management for clients who have been diagnosed with developmental, physical, or intellectual disability. We are contracted with the Department of Mental Health to provide services in Clinton County.
This is professional case management work in a comprehensive purchase-of-service, community supports, and residential placement program for individuals with developmental disabilities.

EXAMPLES OF WORK
Manages a caseload of individuals as assigned by the Executive Director.
Advocates for the individual receiving services and their families
Connects individuals and families with community resources
Assists in scheduling the annual planning meeting
Interviews individuals and their families to develop an annual individual service plan which is a living document that can be changed at anytime
Develops long-term and short-term goals and writing the associated outcomes
Prepares utilization review budgets and arranges for individuals to access services.
Maintain quarterly phone contact with the individual and/or family
Review monthly summaries from service providers
Complete Individual Support Plan reviews monthly of authorized residential services
Complete Individual Support Plan reviews quarterly of authorized in-home services
Complete Individual Support Plan reviews quarterly for individuals with Medicaid
Attends interdisciplinary team meetings to discuss cases in relation to eligibility, support plans, progress, and possible changes in the service program.
Requests modifications for a non-productive program or enrolls the individual in an appropriate program.
Attends staff meetings concerning new or revised policies and procedures.
Visits service agencies, attend meetings, and confers with other support coordinators to become informed concerning community resources for persons with intellectual/physical/developmental disabilities.
Receives close supervision from the Executive Director.
Performs other related work as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
Intermediate knowledge of a specific discipline such as social work, psychology, special education, counseling, health care, or occupational therapy.
Intermediate knowledge of case management methods, principles, and techniques.
Introductory knowledge of various developmental disabilities and corresponding vendors and services available for individuals.
Ability to manage a caseload of individuals with intellectual/physical/developmental disabilities, to keep support plans current, and to maintain accurate records.
Ability to collect and analyze information to make decisions concerning an individual's support plan.
Ability to develop a logical, feasible, and practical support plan for individuals with developmental disabilities.
Ability to evaluate the progress of individuals and the quality of their service programs.
Ability to communicate effectively.
Ability to enter professional and concise case documentation to ensure the continuation of care is accurate
NECESSARY SPECIAL REQUIREMENT
Excellent writing, grammar, and organizational skills
Documentation is an essential component of this position and requires attention to detail and time management skills.
Ability to consistently meet timelines for task completion.
Ability to work a flexible schedule to accommodate service monitoring and/or planning when the individual is available.
Ability to work independently
Ability to meet monthly billing expectations
Operation of a motor vehicle is required & includes proof of a valid driver’s license, the ability to maintain automobile liability insurance as required by MO state law.
EXPERIENCE AND EDUCATION
A Bachelor's degree from an accredited college or university with a minimum of 24 earned semester hours of humanities. One year of experience as an RN, in social work (and related fields), vocational rehab, Special Education, or counseling-related field or direct care to a person with ID/DD.
Two years of case management experience is preferred.

Pay: $41,600.00 per year
