We are seeking a Mobile Team Coordinator in Middlesex, NJ. The Mobile Team Coordinator provides operational support and case management as part of a team that assists children and youth and their families through crises to success.

We are offering a $1,500 initial sign-on bonus opportunity for New Hires! (Must complete 90 days of satisfactory employment to be eligible). New hires will be eligible for a subsequent bonus of $1,500 after 9 months of satisfactory employment. New hires will be eligible for Day 1 benefits upon hire!

Starting pay is $50,000/ annually

Responsibilities

• Supervise team and schedules
• Oversee daily operational aspects of the program
• Arrange and participate in family orientation (within the first 24 hours)
• Assess case management needs of youth and family and provide case management as appropriate (not within the scope of the care management organization)
• Coordinate with the care management organization and CFT as needed
• Provide on-site family psycho-educational activities consistent with the comprehensive treatment and discharge plan

Minimum Requirements

• Ability to establish a comfortable and supportive relationship with individuals receiving supports
• Ability to exercise good judgment and remain calm in crisis situations
• Excellent organizational skills
• Bachelor’s level practitioner with 3+ years of supervisory experience and experience with youth with IDD; OR
• An unlicensed master’s level practitioner with 1 year experience with youth with IDD

Working Conditions

• Required to share on-call responsibilities to ensure around the clock crisis response
• May be exposed to various medical conditions and communicable diseases
