About this position

Blaze Health is seeking a dynamic, experienced urgent care physician to join our urgent care team at Elk River, Maple Grove, Blaine, and Minnetonka clinics. Our urgent care physicians provide treatment to all ages and families who come to the Urgent Care Center. In addition, the Physician acts as part of the treatment team, and ensures the delivery of quality care and maintains professional competence.

Education

• Graduate of an accredited Medical or Osteopathic School

• G2 or beyond in residency training.

Licensure/Certification(s) Mandatory

• Board certified or Board eligible in area of specialty

• Current State of Minnesota Medical Doctor or Doctor of Osteopathy license

• Valid DEA Registration

• Current medical staff credentials for level of training
