WOUND CARE NURSE PRACTITIONER

WOUND CARE NURSE PRACTITIONER POSITION SUMMARY:
Performs skin rounds within the building, develops skin care program, skin champions and performs PI activities. Provides referrals to the wound care center for patients being discharged with pressure ulcers that need outside care. Provides and manages continuous, medical care service to patients under direct consultation with the Medical Director of Diabetic and Wound Care Center, and in collaboration with the private medical attending as indicated. Responds to the requests of the nursing staff as required to ensure patient care and safety. NP must be certified in wound care and maintain current collaborative practice agreement with the Medical Director of the center.

WOUND CARE NURSE PRACTITIONER POSITION REQUIREMENTS:

Education, Licensure & Certifications:
Successful completion of Nurse Practitioner or Physician Assistant program from an accredited school required. NYS Certified RN – NP (Nurse Practitioner) or NYS Certified PA (Physician Assistant). Trained in adult medicine, th Wound Care Certification required. ACLS & BLS certified.

Experience:
Recent patient case management experience in wound care and diabetes; hospital experience preferred. Knowledge of legal and ethical standards for the delivery of medical care needed.
